The following organizations are partners of Medical Care and Outcomes in our three year Federal
Rural Health Outreach Grant: A Collaborative Model for Continuity of Care in Indiana Rural
Health Clinics.

Family Health Services is a private not-for-profit provider of primary health care. Their
mission is to serve the uninsured residents of Rush, Fayette and Franklin counties in east central
Indiana. To accomplish this, the agency is funded in-part by Title V, MCH funds and CHC dollars. |

Hoosier Uplands is a private non-profit health care and community development organization
based in south central Indiana responsible for administering federal and state contracts that
support projects in a five county primary service area and statewide. The organization serves as
a Community Action Agency, Area Agency on Aging, Home Health Care and Hospice Agency, Community
Housing Development Organization, and Area Health Education Center. |

The Indiana Rural Health Association is a not-for-profit organization representing a diverse
statewide membership consisting of individuals and organizations committed to the improvement of
health and resources for rural Hoosiers. IRHA seeks to enhance the health and well-being of rural
populations in Indiana through leadership, education, advocacy, and collaboration. |
